"Ellansé M 1ml is a collagen-stimulating dermal filler designed for long-lasting facial rejuvenation and contouring. Ellansé is unique compared to traditional hyaluronic acid fillers because it’s made of polycaprolactone (PCL) microspheres suspended in a gel carrier (carboxymethylcellulose). The “M” version of Ellansé indicates its longevity: Ellansé comes in S, M, L, ats, each denoting how long the results last (S ~1 year, M ~2 years, L ~3 years, E ~4 years). Thus, Ellansé M provides correction that can last around 2 years before fully metabolizing. Upon injection, Ellansé M immediately provides volume and fills wrinkles, but its true power is in stimulating the body to produce its own collagen around the microspheres. Over time, as the carboxymethylcellulose gel is absorbed, new collagen strands form, maintaining the volume and youthful structure in the treated area for an extended period. Ellansé M’s composition is proprietary, but the key components are: 

  • Polycaprolactone (PCL) Microspheres: These are smooth, spherical biodegradable polymer particles. Inthe PCL microspheres are of a specific size range (about 25-50 microns) which is ideal for stimulating fibroblasts (collagen-producing cells) while avoiding phagocytosis (too small particles would be eaten by macrophages quickly, and too big might cause granulomas). PCL is a well-known bioresorbable medical polymer thatowly via hydrolysis. The concentration of PCL in Ellansé is significant enough to induce a strong collagen response. As time passes, collagen fibers form around each microsphere, effectively “replacing” the volume as the carrier gel dissipates. The PCL in Ellansé M is formulated to last approximately 2 years before completely breaking down into CO2 and water, at which point the newly formed collagen takes over in providing the structural support.
  • Carboxymethylcellulose (CMC) Gel Carrier: The PCL beads are homogeneously suspended in s gel made of CMC (a polysaccharide). The CMC provides immediate volumization upon injection – it’s the initial filler effect that fills wrinkles and lifts areas. It also ensures the product is smoothly delivered and evenly distributes the microspheres. The CMC gel itself typically lasts a few months (perhaps 2-3 months) before being resorbed. By that time, the process of neocollagenesis has begun thanks to the presence of PCL. The gel’s presence also helps keep the microspheres in place at the injection site initially.
  • Sodium Carboxymethylcellulose & Glycerin in a Buffered Saline: The exact formulation may include glycerol for viscosity and longevity of the gel, and phosphate-buffered saline to match physiological pH and osmolarity. These components make the filler smooth and ensure patient comfort (minimal swelling due to iso-tonicity).
  • No Free Monomers or BDDE: Unlike HA fillers that are crosslinked with BDDE, Ellansé’s PCL is polymerized in manufacturing. It’s worth noting that Ellansé does not contain any animal or human components and doesn’t rely on crosslinked HA, thus there is no hyaluronidase remedy – it’s not dissolvable like HA fillers. Its safety profile is good, and because PCL is bioresorbable and the microspheres are smooth (non-angular), the risk of granulomas is low when injected properly.
  • Lidocaine: Actually, Ellansé products do not come pre-mixed with lidocaine (at least not originally, though a newer version with lidocaine might exist, originally they required adding if needed). Typically, the physician may mix lidocaine with Ellansé manually prior to injection or use separate local anesthetic because Ellansé M 1ml syringe in its pure form does not contain lidocaine.

So in summary, the product contains 30% polycaprolactone microspheres (approximately) and 70% CMC gel carrier (with water, glycerin, etc.), although the exact ratio is proprietary, it's around that order. There are differences in molecular weight of PCL or chain length that distinguish S, M, L, E in terms of longevity.

  • Deep Wrinkle Filling: Such as nasolabial folds, marionette lines, and deep chin creases. Ellansé M will fill these immediately and keep them smoother for about 2 years, while encouraging collagen to firm the area from within.
  • Facial Contouring and Volume Restoration: It can be used for mid-face augmentation (like adding volume to cheeks), chin and jawline enhancement (creating a more defined jaw or stronger chin), and even non-surgical rhinoplasty (though in nose, caution is needed as with any filler for vascular reasons, but it’s been done). It’s particularly good for areas where longevity is desired, e.g., a patient who wants a chin augmentation but not an implant might use Ellansé to “try out” the result for couple years. The benefit is that over time, as PCL resorbs, some collagen remains, so they might not lose 100% of the effect even when 2 years are up – a portion of the new collagen can persist beyond the lifespan of the filler.

  • Patients Who Have Tried HA Fillers and Want Longer Duration: Some patients graduate to Ellansé after being comfortable with fillers but frustrated by their short duration. Ellansé M is a good middle ground giving ~2 year result.
  • Collagen Deficient Skin: Since it induces collagen I and III formation, someone whose skin is atonic (low elasticity) can benefit from the improvement in skin quality around the injection area. Over the two years, the treated areas may actually look better than baseline even after filler absorption.
  • Avoiding Overfilling “Pillow face”: Ellansé tends to integrate very naturally and doesn’t attract water like HA can, so there’s minimal risk of that overly puffy look if properly placed. For a patient wanting very natural, undetectable enhancements, a biostimulator is often chosen.

Ellansé M is not intended for areas where very fine detail adjustments are needed that might require dissolving if not perfect (like lip augmentation is typically not done with Ellansé because lips often need nuanced shaping and patients might want it reversible or updated frequently). It’s also not for superficial fine lines due to the risk of visibility of product – it’s placed in deeper dermis or subcutaneously usually. Because it’s longer lasting, practitioners must be skilled in placement – you want to be confident in the injection because it will be there a long time. The product is also firm (gives good structural support, more like Radiesse or Sculptra type effect than the softer fill of some HAs). Patients will appreciate that one syringe can often achieve what might take multiple HA syringes (cost-wise it can be efficient since it lasts long and often one syringe can improve nasolabial folds both sides, for example). 

The typical patient is in good health, moderate volume loss or folds, perhaps not a good candidate for or doesn’t want surgery, and can commit to aftercare (like massage is usually not required as with Sculptra, but the injection technique itself ensures even distribution). Patients will see immediate changes, but also an improvement over 3-4 months as collagen builds. Ellansé M provides a balance of immediate gratification and long-term benefit, which is a big part of its appeal.
